This case report highlights a rare dermatological condition, granular parakeratosis (GP) of the eccrine ostium, in a 64-year-old Thai woman who was misdiagnosed for nearly 40 years. GP presents as brownish to erythematous papules, often mistaken for eczema or folliculitis due to its benign nature. The patient's condition improved with advice to avoid excessive heat and keep affected areas dry.
