TLDR Check hormone levels in older women to find hidden conditions like certain tumors.
This case study described an 88-year-old woman who presented with symptoms of flushing, hirsutism, voice changes, and alopecia, alongside elevated testosterone levels, which were linked to a well-differentiated Sertoli-Leydig cell tumour (SLCT) in her left ovary. SLCTs are rare, androgen-secreting tumors typically found in younger, premenopausal women, making this case particularly unusual due to the patient's advanced age. The findings suggested that it might be beneficial to assess androgen and estrogen levels in postmenopausal women, even those without classical signs of virilization, to identify potential underlying conditions like SLCT.

Elevated bile acids can inhibit the enzyme AKR1C2, leading to increased DHT levels, which may accelerate hair loss in those predisposed to androgenetic alopecia. Treatments mentioned include topical minoxidil and finasteride.
