This report describes a case of primary cutaneous follicle centre cell lymphoma presenting as hair thinning on the scalp, which is not a recognized feature of the condition.

In this case study, a 24-year-old woman with frontal alopecia resembling frontal fibrosing alopecia was ultimately diagnosed with primary cutaneous follicle center lymphoma, highlighting the critical role of biopsy in identifying atypical alopecia presentations.

This review analyzed literature on primary cutaneous lymphomas involving the scalp, finding that scalp involvement in these cases often leads to non-scarring focal alopecia and may indicate more aggressive disease.

PCFCL may have unrecognized subtypes and needs more research.
