- Does metformin help control DHT levels that trigger androgenic alopecia? Metformin has been shown to lower circulating androgens in women with PCOS, but no published study has measured scalp DHT, 5-alpha reductase activity in the follicle, or hair density in anyone taking it — so its effect on pattern hair loss is untested, and it is not a DHT blocker.
- Can metformin improve scalp health in people with insulin resistance or PCOS? There is no controlled trial showing that metformin improves scalp hair. The PCOS studies most often cited measured unwanted body hair, not scalp density, and a published case report describes hair loss during metformin treatment; any use for hair would be off-label and a decision for a prescribing doctor.
