September 2025 in “Clinical Cosmetic and Investigational Dermatology” This systematic review assesses the safety and effectiveness of mesenchymal stem cell (MSC)-derived exosome therapy for hair growth, focusing on androgenetic alopecia (AGA) and other hair loss types. It includes 11 studies with 298 patients, featuring various study designs. Exosomes from sources like adipose tissue, placenta, and foreskin showed improvements in hair density and thickness, with high patient satisfaction and minimal adverse effects. Notably, adipose-derived exosomes, especially when combined with microneedling, demonstrated significant efficacy. Despite promising results, the evidence is limited by study heterogeneity and small sample sizes, indicating a need for more standardized, high-quality randomized controlled trials to confirm these findings and establish treatment protocols.
