White piedra is a fungal infection of the hair shafts caused by Trichosporon species, characterized by white nodules that can increase hair fragility. A 32-year-old female was diagnosed with white piedra caused by T. ovoides using a combination of clinical examination, trichoscopy, microbiologic tests, and molecular methods. Trichoscopy served as a useful tool, bridging the gap between clinical and microbiologic diagnosis and eliminating the need for hair shaft microscopy. The Trichosporon genus includes 6 clinically significant species, which are morphologically indistinguishable but can be identified genotypically. The patient was successfully treated with oral itraconazole and topical ketoconazole.
