TLDR Ulcerative sarcoidosis in body folds is rare and improved with prednisone and hydroxychloroquine.
This case study describes a rare presentation of ulcerative sarcoidosis (US) in a woman in her 40s, with lesions located exclusively on body folds and associated with scalp sarcoidosis (ScS). The patient exhibited painful ulcerative plaques and systemic symptoms, including arthralgia and splenomegaly. Diagnostic challenges arose due to the atypical presentation and the need to differentiate from other conditions like metastatic Crohn's disease. Histological analysis revealed non-caseating granulomas, supporting the sarcoidosis diagnosis. Treatment with prednisone and hydroxychloroquine led to improvement. The study highlights the rarity of US in folds and the diagnostic value of trichoscopy in ScS.
