TLDR Shiny white structures in trichoscopy can indicate long-standing discoid lupus erythematosus alopecia.
The study systematically described trichoscopic findings in 37 patients with discoid lupus erythematous alopecia, analyzing 55 lesions. The majority of patients were women with a mean age of 54.4 years. Trichoscopy revealed that patients with shorter disease duration showed incontinentia pigmenti signs and thin vascular structures more frequently, while long-standing lesions had an increased number of shiny white structures, which were suggested as a diagnostic clue for long-standing lesions. The study highlighted the presence of uncommon trichoscopic findings such as red dots and "red spider on yellow dots." The findings were not significantly influenced by treatments received.
