TLDR Combining isotretinoin and dapsone effectively treated a challenging scalp condition.
Perifolliculitis capitis abscedens et suffodiens (PCAS) is a challenging scalp condition often associated with scarring alopecia and other skin issues. A 19-year-old male patient with a 2-year history of PCAS did not respond to various antibiotic treatments or surgery. Initial monotherapy with isotretinoin was ineffective. However, a combination therapy of isotretinoin (80 mg daily) and dapsone (100 mg daily) led to significant improvement within 4 weeks. After 6 months, dapsone was reduced to 50 mg daily, and isotretinoin was gradually discontinued. The patient continued on dapsone 50 mg every other day and remained symptom-free for 6 months.
