TLDR Consider THPP in patients with muscle weakness and low potassium, as it is often underdiagnosed.
A 49-year-old Brazilian male with hyperthyroidism experienced worsening muscle weakness, tremors, and hair loss, alongside other symptoms, despite treatment with methimazole. He was diagnosed with thyrotoxic hypokalemic periodic paralysis (THPP), characterized by low potassium levels and muscle weakness, which is more common in males at a 20:1 ratio. The case highlights the importance of considering THPP in patients with muscle weakness and low potassium, as it is often underdiagnosed due to nonspecific symptoms. Potassium replacement should be approached cautiously.
