TLDR Mesenchymal cells are essential for hair follicle stem cell growth.
The study investigated the location and proliferation of human hair follicle stem cells, identifying two distinct stem cell reservoirs in the hair follicles: one in the bulge area and another in the outer root sheath above the hair bulb. The research found that stem cell proliferation occurred only when the follicle epithelium was co-cultured with mesenchymal cells, specifically dermal papilla cells. The study concluded that mesenchymes are essential for stem cell proliferation, and the resulting colonies contained juvenile cells, mature keratinocytes, and apoptotic cells, indicating different stages of stem cell development.

Treatments for hair loss, including finasteride, dutasteride, minoxidil, ketoconazole, microneedling, and low level laser light therapy, which aim to reduce DHT production, increase cell absorption and blood flow, and stimulate epidermal stem cells. It also stresses the importance of patience when using these treatments.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

PP405 is discussed as a potential hair growth stimulant, possibly more effective than minoxidil, but not a cure for hair loss. There is skepticism about its ability to regrow "deadzones," and concerns about its impact on those with hair transplants.

Increased Malassezia and Cutibacterium in the scalp microbiome are linked to higher sebum production and inflammation in androgenetic alopecia (AGA). Treatments include ciclopirox shampoo, benzoyl peroxide shampoo, clobetasol propionate, calcipotriol, minoxidil, finasteride, and dutasteride.
