TLDR Remote monitoring of chronic diseases like dermatomyositis is challenging and needs better tools for early detection and patient education.
During the COVID-19 pandemic, a 36-year-old female with dermatomyositis experienced a relapse after switching from injectable methotrexate to oral medication due to lockdown restrictions. Teleconsultation revealed symptoms such as difficulty climbing stairs, periungual skin changes, and diffuse hair loss. Remote assessments, including a 2-minute walk test and arm lifting, indicated reduced physical capability, while elevated muscle enzymes confirmed the relapse. The case highlighted the challenges of remote monitoring in chronic diseases, emphasizing the need for simple, accessible tools for early detection and patient education on sharing clinical images effectively.
