Immunohistochemical Analysis of T-Cell Subsets in the Inflammatory Infiltrates of Alopecia Areata and Its Comparison with Androgenetic Alopecia

This study conducted at Shohadae-Tajrish Hospital in 2018 compared T-cell subsets in the inflammatory infiltrates of alopecia areata (AA) and androgenetic alopecia (AGA) using immunohistochemical analysis. It involved 28 AA cases and 32 AGA cases. The results showed that peribulbar lymphocytic infiltration was significantly more common in AA patients (88.5%) compared to AGA patients (12.5%). Additionally, higher densities of CD3, CD4, and CD8+ T-cells were observed in various skin regions of AA patients, while CD3 and CD4+ T-cells around sebaceous ducts were more indicative of AGA. The study concluded that peribulbar lymphocytic infiltration and T-cell distribution in skin tissues are crucial for differentiating AA from AGA with high precision.
