TLDR Hair loss without scarring is more common than permanent hair loss with scarring, and is often due to genetic factors.
Non-scarring (non-cicatricial) alopecia, which can be caused by androgenetic alopecia, alopecia areata, telogen effluvium, or anagen effluvium, is more common than scarring alopecia, which results in permanent hair loss and skin scarring due to damaged hair follicles. Androgenetic alopecia is the most prevalent cause of non-scarring alopecia, affecting up to 50% of women and 80% of men, with the incidence increasing with age post-puberty. The clinical manifestations of androgenetic alopecia vary between sexes. Diagnosis primarily relies on trichoscopy, which has replaced scalp biopsy. Side effects of treatment, although rare and reversible, can include sexual issues such as loss of libido, erectile dysfunction, and ejaculation problems.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
