TLDR Environmental factors can trigger alopecia areata in identical twins.
This case report documents the first instance of simultaneous alopecia areata (AA) in identical twins in Saudi Arabia. The 6-year-old monozygotic twin girls presented with identical patchy hair loss on the vertex of their scalps, discovered within a 2-week interval. Despite no family history of AA or related autoimmune disorders, the twins exhibited identical noninflammatory bald patches. Treatment with topical clobetasol propionate was successful. The report highlights the rarity of simultaneous AA in identical twins and suggests further research into genetic and environmental factors contributing to AA's development.
