This case report discusses a 15-year-old girl with a nevus sebaceus of Jadassohn (NSJ) on her scalp, which developed a secondary neoplasm. Initially appearing as a yellowish plaque at age 5, the lesion grew and developed a bluish-gray nodule with pain. Examination and histopathology confirmed NSJ with features suggesting a pigmented trichoblastoma or possibly a pigmented basal cell carcinoma. The report emphasizes the importance of early recognition and evaluation of NSJ lesions, especially during adolescence, to detect secondary neoplastic transformations and guide management, including potential surgical excision.
