Recurrent Priapism After Initiation of Oral Minoxidil

TLDR Oral minoxidil may cause priapism, especially in people with certain health conditions.
A 46-year-old male with end-stage renal disease, hypertension, type 2 diabetes, and sickle cell trait experienced recurrent priapism episodes after starting oral minoxidil for hypertension. Despite treatment with finasteride, the episodes continued until minoxidil was discontinued, after which no further episodes occurred. This case suggests a potential link between oral minoxidil and priapism, especially in patients with predisposing factors, highlighting the need for awareness of this possible side effect in similar clinical scenarios.
