Primary scarring alopecias are scalp disorders leading to permanent hair loss due to hair follicle destruction and replacement by scar tissue. The review highlights the importance of early and accurate identification for effective management and prevention of further damage. It discusses the complex etiology involving genetic factors, autoimmunity, chronic inflammation, and disruptions in the hair follicle cycle. Recent findings indicate alterations in skin bacterial composition in conditions like lichen planus pilaris and frontal fibrosing lichen planus pilaris. Despite advancements, differential diagnosis remains challenging due to overlapping clinical and pathological features.
