    Primary essential cutis verticis gyrata (CVG) is a rare, benign scalp condition characterized by excessive skin and subcutaneous tissue growth, forming symmetrical folds and furrows. This case report describes a woman in her late thirties with asymptomatic scalp thickening, diagnosed with primary essential CVG after extensive testing ruled out secondary causes. Histopathology showed hyperplasia of the pilosebaceous unit and hair follicles. Primary essential CVG is extremely rare in females, with only three reported cases. Treatment focuses on symptom management and maintaining local hygiene, with surgery considered for cosmetic or psychological reasons.
