Primary essential cutis verticis gyrata (CVG) is a rare, benign scalp condition characterized by excessive skin and subcutaneous tissue growth, forming symmetrical folds. This case report describes a woman in her late thirties with asymptomatic scalp thickening and folds, diagnosed as primary essential CVG after ruling out secondary causes through comprehensive examinations and a biopsy. Primary essential CVG is extremely rare in females, with only three reported cases. Treatment focuses on symptom management and maintaining hygiene, with surgical options for cosmetic or psychological concerns. The diagnosis requires excluding primary non-essential and secondary CVG causes.
