TLDR Young transgender, gender diverse, and nonbinary people on spironolactone have a low risk of high potassium, so frequent monitoring may not be needed.
This study evaluated the necessity of frequent potassium monitoring in 318 transgender, gender diverse, and nonbinary individuals on spironolactone, focusing on hyperkalemia incidence and age influence. It found a low hyperkalemia risk (1.5%) in those aged 45 or younger, suggesting that frequent monitoring might be excessive for this group, potentially reducing healthcare costs without compromising safety. However, individuals over 45 or with certain medical conditions should continue monitoring. The study, the largest of its kind, noted limitations due to its single-institution setting and predominantly Caucasian cohort, which may affect result generalizability.
