A Rare Case of Postmenopausal Hirsutism Associated With a Serous Cystadenofibroma of the Ovary

    TLDR Benign ovarian tumors can cause excess male hormones and related conditions in postmenopausal women.
    A 54-year-old postmenopausal woman with hirsutism and scalp hair loss was diagnosed with a benign ovarian serous cystadenofibroma. Despite normal initial imaging, further tests revealed a solid-cystic ovarian structure and endometrial thickening. Post-surgery, which included a total hysterectomy and bilateral salpingo-oophorectomy, her hirsutism resolved and testosterone levels normalized. This case underscores the importance of evaluating adrenal or ovarian sources in postmenopausal hirsutism, as benign ovarian tumors can cause androgen excess and related conditions like endometrial cancer.
