A 19-year-old woman with systemic lupus erythematosus (SLE) presented with symptoms including fever, joint pain, photosensitivity, oral ulcers, and hair loss, and was admitted to Bangabandhu Sheikh Mujib Medical University in Dhaka, Bangladesh. She later developed progressive anemia and dark-colored urine, and flow cytometry confirmed the presence of a PNH clone within her red cells. This case highlights the rare occurrence of paroxysmal nocturnal hemoglobinuria (PNH) in a patient with SLE, emphasizing the need for clinicians to be aware of this possible association.
