TLDR Young patients with Vogt-Koyanagi-Harada syndrome may have poor vision outcomes despite treatment due to frequent recurrences.
A 17-year-old female with Vogt-Koyanagi-Harada syndrome experienced multiple ocular manifestations and recurrences, including bilateral macular serous detachment and anterior uveitis. Despite initial successful treatment with oral prednisolone, eye drops, and sub-Tenon's triamcinolone injections, which restored her vision to 20/20, her condition led to atrophic scars at the fovea, resulting in a final visual acuity of 20/600 in the right eye and 20/50 in the left eye. The case highlighted the importance of gradual steroid tapering and the potential for poor visual outcomes in young patients with frequent recurrences.
