A 49-year-old man with a therapy-resistant lesion on his forearm was diagnosed with botryomycosis, characterized by an erythematous, verrucous, and erosive lesion. Despite initial unsuccessful treatments with topical ointments and oral therapies, histopathology confirmed the infection through the Splendore-Hoeppli phenomenon. The condition responded extremely well to long-term oral antibiotics.
