TLDR A woman with severe hair loss regrew mostly white hair after treatment.
This case study reported on a 20-year-old woman with diffuse alopecia areata, characterized by the loss of more than 80% of her scalp hair, sparing non-pigmented hairs. After a 6-month treatment with oral prednisolone and methotrexate, she experienced extensive regrowth of non-pigmented (white) hair, resulting in a normal hair density but with white hair. This response was sustained for 4 months after stopping treatment. The phenomenon of widespread non-pigmented hair regrowth in diffuse alopecia areata was attributed to the selective loss of pigmented hairs, possibly due to antibodies targeting melanocytes, and reduced melanoblasts and abnormal melanogenesis in regrowing areas.
