TLDR Multiple pilonidal sinuses can be successfully treated with a mix of surgery and conservative care.
This case report and literature review discuss an exceptionally rare occurrence of multiple concurrent pilonidal sinuses (PNSs) in a 19-year-old female, presenting with sinuses in the intermammary, umbilical, and sacrococcyx regions. The intermammary PNS (IMPNS) was surgically excised, leading to complete healing, while the natal cleft PNS (NCPNS) was treated surgically using Lord’s procedure, and the umbilical PNS (UPNS) was managed conservatively. Histopathology confirmed chronic sinus tract formation in all cases. The review of nine additional PNS cases highlighted the rarity of such concurrent presentations and emphasized that a combination of surgical and conservative management can yield favorable outcomes.
