This document reports a rare case of pilonidal sinus in the preauricular region, marking only the second such case in the literature. It describes a 10-year-old boy with recurrent discharge from this area, treated successfully through surgical excision. Histopathological examination confirmed the presence of a sinus tract lined by epidermis and filled with keratin and hair shafts. The report underscores the importance of complete excision and regular follow-up to prevent recurrence and highlights the unusual occurrence of pilonidal sinus outside the sacrococcygeal region, particularly in the auricular area.
