New-Onset Majocchi’s Granuloma in Two Kidney Transplant Recipients Under Tacrolimus Treatment

TLDR Majocchi's granuloma can occur in kidney transplant patients on tacrolimus and can be treated with antifungal medication.
The document reported two cases of Majocchi’s granuloma in kidney transplant recipients treated with tacrolimus, a potent immunosuppressant. Both Hispanic men, aged 53 and 60, developed skin lesions post-transplantation. The first patient had violaceous plaques with pustules that resolved after 12 weeks of terbinafine treatment, following ineffective itraconazole therapy. The second patient had purple papules and nodules that improved after 3 months of itraconazole, though some skin changes remained. The study highlighted the need for dermatologists to consider Majocchi’s granuloma in immunosuppressed patients, despite tacrolimus being more commonly associated with bacterial and viral infections.
