Long-Term Pubic Dermatitis Diagnosed as White Piedra

    TLDR A long-term pubic rash was finally diagnosed as white piedra and cured with ketoconazole.
    A 58-year-old man experienced a pruritic rash in the pubic area for 30 years, which went undiagnosed despite evaluations by at least 15 different primary care physicians and dermatologists. Multiple treatments failed to resolve the issue. A Wood lamp evaluation eventually revealed hair shaft concretions, which were confirmed as white piedra (WP) through histologic evaluation. The patient was successfully treated with topical ketoconazole, leading to complete resolution of the eruption. This case highlighted the importance of using Wood lamp and dermoscopy to diagnose parasitic infections of the pubic hair shafts in cases of nonspecific dermatitis.
