    A 44-year-old male with pulmonary tuberculosis developed blue-gray skin discoloration and alopecia areata with ophiasis pattern 20 days after starting levofloxacin as a second-line treatment. Levofloxacin was identified as the likely cause due to the timing of the skin changes and was discontinued. The patient's skin discoloration and other symptoms resolved gradually after stopping the drug. This case suggests that skin discoloration and alopecia could be potential adverse effects of levofloxacin. Clinicians should be aware of these potential complications for appropriate diagnosis and treatment.
