TLDR The document does not confirm if radical prostatectomy is the best treatment for locally advanced prostate cancer.
The document reviews several studies related to prostate cancer treatments but does not definitively conclude whether radical prostatectomy is the best option for locally advanced prostate cancer. One case-control study involving 29 opium users and 39 non-users found a significant link between opium consumption and lower PSA levels. Another study with 358 men showed a 51.4% positive predictive value for PSA tests in detecting malignant prostate lesions. A prospective study with 194 cases and 317 controls found no significant correlation between serum triglyceride levels and prostate cancer risk. A study of 45 patients post-radical prostatectomy indicated a relationship between higher urethral PSA, higher androgenic alopecia scores, and higher postoperative PSA. An 11-man study comparing anastomotic stricture rates after radical retropubic prostatectomy found no results in the summary. Another study with a median follow-up of 26 months found Velthoven's running suture technique to be safe and reliable. A comparison between open retropubic and laparoscopic prostatectomy in 330 patients from Bulgaria and Germany highlighted the importance of patient selection and healthcare quality, with the German clinic showing better outcomes. Lastly, a multi-institutional study with 282 Bulgarian patients validated nomograms for predicting pathological stage and recurrence after radical prostatectomy.
