A 27-year-old woman developed a swelling in her left parotid gland, which was diagnosed as an epidermoid cyst through MR imaging and ultrasonography. The cyst appeared after she received a botulinum toxin injection in her left masseter muscles, with swelling noticed four months post-treatment. The cyst was surgically removed and found to be encapsulated by a thin wall and filled with keratin. The final diagnosis confirmed it as an epidermoid cyst, a condition typically treated by surgical excision with uncommon recurrence.
