Iatrogenic Androgenetic Alopecia in a Male Phenotype 46XX True Hermaphrodite

    This article describes a unique case of a castrated male phenotype 46XX true hermaphrodite receiving exogenous androgen supplementation who developed male-type hair loss. The patient was diagnosed with iatrogenic androgen-induced alopecia and was treated with finasteride, which stabilized the hair loss and resulted in scalp hair regrowth. The case serves as a reminder that iatrogenic androgenetic alopecia should be considered in patients on testosterone replacement therapy and supports an androgenic aetiology in women with AGA. Furthermore, the article suggests that Hamilton type alopecia could be a positive predictive marker of finasteride therapy in female patients.
