TLDR A young man with high-grade prostate cancer had successful surgery and good recovery, highlighting the need for awareness in younger men.
A 35-year-old male, using finasteride for hair loss, was diagnosed with high-grade prostate cancer after presenting with an elevated PSA level of 12.5ng/mL. A biopsy revealed Gleason 4+5=9 acinar adenocarcinoma, and subsequent robotic-assisted radical prostatectomy showed the same Gleason score with extraprostatic extension but clear margins, classified as stage pT3a N0 M0. At 12 months post-surgery, the patient had undetectable PSA levels, immediate continence, and good erectile function. This case underscores the importance of considering prostate cancer in younger men below the typical screening age, as early surgical intervention can lead to favorable functional and oncological outcomes, despite the absence of family history or positive genetic testing for prostate cancer.
