Hair Transplantation on the Baldness Region with Free Latissimus Dorsi Flap for Scalp Reconstruction: A Case Report

TLDR Hair transplantation on a reconstructed scalp can effectively improve hair growth and appearance.
This case report discusses a novel approach to scalp reconstruction in a 44-year-old woman with a history of Moyamoya disease and subsequent scalp defects due to craniotomies and infections. A free myocutaneous latissimus dorsi flap was used to reconstruct a 13 × 9 cm scalp defect. Nine years later, a 1,500-unit follicular unit extraction (FUE) hair transplantation was performed on the flap, achieving an 80 to 85% hair survival rate at 12 months. The procedure significantly improved the patient's alopecia and aesthetic satisfaction, demonstrating that FUE hair transplantation on a myocutaneous free flap can effectively address alopecia and enhance aesthetic outcomes in scalp reconstruction.
