Characterization of Scalp, Eyelash, and Eyebrow Hair Regrowth in Patients with Severe Alopecia Areata Randomized to Placebo in BRAVE AA1 and BRAVE AA2 Trials

    TLDR Severe alopecia areata patients have a low chance of spontaneous hair regrowth.
    In the BRAVE-AA1 and BRAVE-AA2 trials, 345 patients with severe alopecia areata (AA) were treated with a placebo for 36 weeks to assess spontaneous hair regrowth. The mean change in the Severity of Alopecia Tool (SALT) score was -5.5, with 4.1% achieving a SALT score ≤20 and 9.3% achieving SALT50 (≥50% improvement). Eyebrow and eyelash regrowth were minimal, with 3.8% and 4.3% of patients showing significant improvement, respectively. Patients with better outcomes generally had a lower baseline SALT score, shorter AA duration, and a current episode of less than 4 years. These findings suggest that spontaneous hair regrowth in untreated severe AA is rare, which is important for setting patient expectations.
