TLDR FFA in young women is often missed, and no single treatment works best.
A 28-year-old premenopausal woman with frontal fibrosing alopecia (FFA) presented with a 1-year history of rash, hair loss along the frontal hairline, thinning eyebrows, and increased skin sensitivity. Examination and biopsy confirmed FFA. She was treated with hydroxychloroquine, topical corticosteroids, minoxidil foam, and intralesional corticosteroid injections. After 3 months, she experienced reduced erythema and symptoms, with no further hair loss. The case highlighted that FFA in young women might be underrecognized and that while various treatments exist, no single regimen has proven consistently effective.
