Folliculosebaceous Cystic Hamartoma With Spindle Cell Lipomatous And Neural Components

    This report describes a rare case of folliculosebaceous cystic hamartoma (FSCH) in a 55-year-old woman, presenting as a 10 × 9 mm nodule on her nose. The lesion, excised without complications, exhibited a mix of follicular, sebaceous, and mesenchymal tissues, including spindle cell lipomatous and neural components. Histopathological examination revealed a dilated follicular structure with hypertrophic sebaceous glands, mature adipose tissue, and fibromyxoid stroma containing CD34-positive spindle cells, S100-positive neural structures, and SMA-positive vascular structures. The lesion was benign, with no cytologic atypia or mitotic figures. This case underscores the diagnostic complexity of FSCH, particularly with the presence of rare spindle cell lipomatous and neural components.
