A 61-Year-Old Filipino Man with Lichen Planus Concomitant with Cicatricial Alopecia, Mimicking Discoid Lupus Erythematosus

    Image of study
TLDR A 61-year-old Filipino man had a skin condition that looked like another disease, making diagnosis difficult.
A 61-year-old Filipino man presented with persistent erythematous hairless scarring plaques on the scalp and atrophic plaques on the face and upper extremities, initially suggesting discoid lupus erythematosus (DLE). However, a skin punch biopsy revealed histopathological features of lichen planus (LP). This case highlights that LP, particularly its follicular variant lichen planopilaris (LPP), can mimic DLE due to its scarring alopecia and atrophic skin lesions. The study underscores the importance of biopsy in differentiating between these conditions.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
