TLDR Erythema nodosum linked to kerion can be treated effectively with antifungals and steroids to reduce scarring.
This case series and narrative review explore the rare association between erythema nodosum (EN) and kerion, primarily caused by Trichophyton mentagrophytes. The study presents three cases successfully treated with antifungal medications like itraconazole or griseofulvin, often combined with prednisone to reduce scarring risk. A review of 23 cases revealed that EN typically develops after antifungal treatment, with a mean onset time of 11.58 days, and resolves in an average of 8.31 days. Griseofulvin remains a mainstay treatment, with complete kerion response occurring within 6 weeks. Scarring alopecia is common, highlighting the importance of adjunct corticosteroid therapy. The study emphasizes recognizing the link between EN and kerion for effective treatment and suggests further molecular research to explore potential associations between specific genotypes and EN occurrence.
