This case report discusses a 16-year-old male with dermatosis neglecta (DN) mimicking pemphigus foliaceus, associated with obsessive–compulsive disorder. DN is an underrecognized skin disorder resulting from neglect of self-cleaning, leading to the accumulation of skin debris. It presents as yellowish-to-brownish, waxy papules or plaques and can be mistaken for other dermatological conditions. The case highlights the importance of DN awareness to avoid unnecessary diagnostic procedures, as DN lesions can be effectively cleared with proper cleansing and alcohol. The patient's DN was confirmed through histopathological examination, and the lesions resolved after facial cleansing and psychiatric treatment, suggesting DN may indicate underlying psychiatric disorders.
