Lung Is a Battlefield: Diffuse Alveolar Hemorrhage in Antiphospholipid Syndrome

    This case report discusses a rare and life-threatening complication of catastrophic antiphospholipid syndrome (CAPS) known as diffuse alveolar hemorrhage (DAH) in a 41-year-old woman with a history of antiphospholipid syndrome (APS). The patient presented with respiratory failure and hemoptysis, leading to a diagnosis of DAH. Treatment included high-dose glucocorticoids, plasma exchange therapy (PLEX), and daratumumab, resulting in improvement and extubation. The report emphasizes the importance of early recognition and a multidisciplinary approach in managing DAH in APS patients, highlighting the potential role of daratumumab, although its use in severe CAPS is still under investigation.
