Concordance Between Clinician-Reported and Patient-Reported Outcomes of Eyebrow and Eyelash Hair Loss in Patients With Severe Alopecia Areata: Results From BRAVE-AA1 and BRAVE-AA2 Studies

TLDR Patients and clinicians generally agree on eyebrow and eyelash hair loss improvements in severe alopecia areata.
The study evaluated the concordance between clinician-reported outcomes (ClinRO) and patient-reported outcomes (PRO) for eyebrow and eyelash hair loss in 1,191 patients with severe alopecia areata over 52 weeks, using data from the BRAVE-AA1 and BRAVE-AA2 phase 3 studies. Strong positive correlations were found between ClinRO and PRO measures for both eyebrows and eyelashes at baseline, Week 36, and Week 52, with Spearman's rank correlation coefficients ranging from 0.83 to 0.87. Concordance rates were highest for scores indicating no notable hair (3) and full coverage (0), while scores 1 and 2 showed lower concordance, possibly due to clinicians' unfamiliarity with patients' normative appearance. Overall, more than 75% of patients agreed with clinicians on ≥1-point improvement, and approximately 87%-89% agreed on ≥2-point improvement from baseline in both eyebrows and eyelashes by Week 52. These findings suggest that patient assessments using PROs align closely with clinician assessments.
