TLDR Skin problems are common in the elderly, with itching and wrinkling being the most frequent issues.
In 2019, a cross-sectional observational study was conducted on 540 patients aged 60 and above at Era’s Lucknow Medical College and Hospital in India to examine the clinical patterns and frequency of cutaneous manifestations in the elderly. The study found that 55.7% of the patients were males and 44.3% were females. The most common complaint was pruritus, reported by 90.6% of the patients. Among the physiological changes, wrinkling was the most common, observed in 80.9% of the patients. Pathological changes such as skin tumors, eczemas, and infections were also common findings. The study concluded that geriatric dermatoses vary among different populations, and with a significant number of the elderly population, increased emphasis on geriatric medicine is inevitable. The findings of this study aid in the early diagnosis and management of geriatric dermatoses.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

Topical finasteride is considered a safer option for hair regrowth with fewer systemic side effects compared to oral finasteride. Some users report similar efficacy and side effects between topical and oral treatments, while others prefer topical due to reduced systemic exposure.

Kintor Pharmaceutical's Phase 2 results with the drug Pyrilutamide, a potential alternative to 5-ARI drugs like Finasteride and Dutasteride but without the side effects. It could be used in combination with other topical treatments such as Minoxidil. The safety of the drug has been demonstrated in 6 months of use, however there are still questions about long term efficacy and systemic absorption.
