A 79-year-old male with Parkinson’s disease, diabetes, and hypertension presented with a rash resembling erythema multiforme, which was diagnosed as bullous pemphigoid (BP) through biopsy and immunofluorescence. The patient improved with prednisone and triamcinolone treatment. This case is notable as it represents the third reported instance of idiopathic targetoid BP resembling erythema multiforme, highlighting the need for awareness of atypical BP presentations to ensure timely and appropriate treatment.
