A 68-year-old woman with a 13-year history of conjunctival and oral lesions characteristic of benign mucous membrane pemphigoid also exhibited unusual hyperkeratotic plaques on her distal extremities. She had a sore mouth, eye irritation, and persistent thickened plaques on her wrists and lower legs for five years, along with a ten-year history of finger swelling and discolored, opaque fingernails. Examination revealed eyelid hair loss, symblepharon formation, patchy denuded palatine mucosa, atrophic areas on the vaginal labia and external auditory canal, and well-demarcated, symmetrical, furrowed, crusted, firm, tannish-brown plaques on her distal extremities.
