This case report follows a menopausal woman experiencing symptoms of androgen excess, including female pattern hair loss (FPHL), over a 10-year period. Despite initial weight loss and reduced insulin levels, her alopecia and hirsutism persisted. The introduction of cyproterone acetate therapy led to a significant reduction in testosterone levels and clinical improvement in her alopecia, reducing it from Grade 3 to Grade 1. The report highlights the importance of normalizing testosterone levels to mitigate androgenization effects, such as alopecia and hirsutism, and suggests that reducing hyperinsulinemia alone may not be sufficient for these improvements.
