TLDR Anti-androgens help manage PCOS symptoms like excess hair and acne.
This review article discusses the role of anti-androgens (AAs) in managing symptoms of polycystic ovary syndrome (PCOS), such as hirsutism and acne, which are caused by high androgen levels. Common AAs include spironolactone, flutamide, finasteride, and cyproterone acetate, often used with effective contraception to prevent pregnancy. The article examines the mechanisms, clinical effectiveness, safety concerns, and guidelines for these treatments, highlighting the need for further research on long-term safety, metabolic effects, and direct comparisons among different AAs. Future studies aim to improve metabolic health, reduce long-term cardiovascular and metabolic risks, and enhance ovulation rates. Additionally, there is interest in combination treatments for severe hair loss, safe use in adolescents, personalized treatments based on genetics, and new topical forms to minimize systemic risks. Long-term management of persistent hirsutism is also a significant focus.
