A Comparison of Varying Alpha-Blockers and Other Pharmacotherapy Options for Lower Urinary Tract Symptoms

    TLDR Alpha-blockers are the most effective and widely used treatment for lower urinary tract symptoms, with minimal side effects.
    Alpha(1)-adrenoceptor antagonists were established as the most common treatment for lower urinary tract symptoms (LUTS) related to benign prostatic hyperplasia, effective for all patients regardless of prostate size, unlike 5alpha-reductase inhibitors like finasteride, which were only suitable for patients with enlarged prostates. Studies showed alpha(1)-blockers significantly improved urinary flow and reduced symptoms compared to placebo, with maintained efficacy over time and no tolerance development after 6-12 months. In short-term comparisons, alpha(1)-adrenoceptor antagonists were more effective than finasteride in reducing symptoms. Common side effects included dizziness and hypotension, with alfuzosin affecting blood pressure more than tamsulosin, which was well tolerated and had minimal blood pressure effects.
