This document discusses the occurrence of relapses in three patients with severe alopecia areata (AA) undergoing treatment with baricitinib, a JAK 1–2 inhibitor. Despite initial hair regrowth observed in all cases, relapses occurred either within the regrowth areas or in previously unaffected areas, indicating two different relapse phenotypes. The study emphasizes the importance of discussing potential relapses with patients during pretreatment counseling. The long-term clinical response to baricitinib in AA patients remains to be further evaluated. The study involved three female patients aged 28, 43, and 52, with no history of atopy or atopic dermatitis, and treatment was not discontinued despite relapses.
